His mother, Rachel, was calling my name before the ambulance doors were even shut.

Warfarin was a prescription blood thinner. Noah had no medical reason to receive it.

“How recent?”

“Repeated exposure. Not one accidental dose.”

I turned toward Rachel.

“Does anybody in your house take anticoagulants?”

She shook her head far too quickly. “No.”

“Any rat poison inside the house?”
“No. Daniel, what are you suggesting?”

Then Ortiz’s face changed.

“There’s something else,” she said. “They discovered another policy. Same amount. Same beneficiary.”

Rachel looked bewildered. “On who?”

“On you.”

The room suddenly felt smaller.

According to the paperwork, Rachel’s policy had been active for nine months. Officers had also discovered a plastic container beneath the driver’s seat of her vehicle. Inside were crushed tablets matching those recovered from Noah’s stomach.

Mark had not been preparing for one death.

I stared through the glass at Noah, tiny beneath warming blankets and medical tubing.

The basement smelled like detergent, heated metal, and diesel fumes. Fluorescent lights flickered above long rows of linen carts.

At the distant end of the hallway, a portable monitor released one sharp electronic beep.

We followed the noise toward an open supply room.

A respiratory therapist named Colin Price lay on the floor, conscious but bl33ding from his scalp. His hospital badge and scrub jacket were gone.

“He attacked me,” Colin whispered. “Said his child was dying. I turned around, and he hit me.”

“Was somebody helping him?” Ortiz asked.

Colin nodded faintly. “A woman wearing scrubs. Dark hair. I couldn’t see her face.”

That explained how Mark had transported Noah so quickly. He had taken hospital credentials and relied on an accomplice familiar with hospital procedures.

One security officer discovered wheel tracks leading toward the loading dock. Beside them were several small drops of blood, probably from Mark’s injured arm.

We reached the dock just as a white hospital linen van sped toward the exit.

Ortiz did not fire. The van contained a critically ill child, and one bullet could transform a rescue attempt into a tragedy.

Police cruisers pursued instead.

I climbed into Ortiz’s unmarked vehicle without requesting permission.

“You’re a physician, not law enforcement,” she said as she accelerated away.

“I’m the only doctor here who knows precisely what Noah needs.”

She glanced at me once before handing me the radio microphone. “Tell dispatch that.”

The pursuit continued north through deserted Providence streets, never reaching speeds that might force the van into a cr@sh. Officers closed intersections and maintained their distance. Ortiz believed Mark wanted us near enough to watch but too far away to interfere. He wanted control. He wanted an audience.

The van entered an abandoned warehouse district beside the river and v@nished inside a former medical-supply facility.

Police quickly surrounded the building.

Less than a minute later, a text appeared on Rachel’s phone.

COME INSIDE ALONE OR HE DIES.

Ortiz read the message and immediately refused to send Rachel inside. Instead, officers traced it to a prepaid phone somewhere within the warehouse. A tactical team prepared to enter, but I warned them that Noah’s portable oxygen supply might soon be exhausted. His critical treatment had also been interrupted. Every passing minute increased the danger of permanent liver damage or catastrophic internal bleeding.

Then Mark called my phone.

I had never given him my number.

“You always believed you were smarter than everybody else,” he said.

His voice carried a faint echo, suggesting he was somewhere inside a large room.

“What do you want?”

“Rachel.”

“You poisoned a four-year-old just to punish her?”

“So you chose to kill him.”

“You come inside, you bring Rachel.”

“She’s not here.”

“You’re lying.”

“I’m outside the building. You can see the flashing police lights. Let me enter alone.”

Ortiz shook her head, but I held her gaze and pointed toward the building’s western side. As Mark continued speaking, the echo shifted whenever a train passed somewhere nearby. The strongest vibration reached the phone several seconds after the tracks along the eastern wall trembled. That suggested he was farther inside the structure, likely near the loading area facing the river.

Ortiz understood immediately. Without speaking, she redirected the tactical officers.

Mark eventually agreed to let me enter through the front entrance.

An officer secured a transmitter beneath my scrub shirt. I carried a medical bag containing vitamin K, plasma-compatible equipment, oxygen tubing, and Noah’s antidote infusion. Nothing that could be used as a weapon.

Inside, the warehouse was nearly pitch-black. Moonlight filtered through shattered windows high above. Empty shelving created narrow corridors. Somewhere farther inside, water dripped steadily.

“Keep moving,” Mark called out.

I followed the sound of his voice toward a broad loading bay.

Noah was lying on a gurney beside an abandoned ambulance. His skin looked gray. The portable oxygen tank was almost empty. A woman stood behind the gurney, holding a kitchen knife close to his neck.

I knew her.

“There is no ‘we’ unless she comes here,” he replied.

Tessa stared at him. “What is that supposed to mean?”

He turned the pistol in her direction.

At that exact moment, the tactical officers entered through the rear doors.

I rushed directly to Noah.
